In-Warranty Hearing Aid Repair (2 Hearing Aids)

PLEASE READ ENTIRE DESCRIPTION AND REPAIR FORM BEFORE SUBMITTING IT TO HEARING AID SERVICES PLLC.

This repair is for TWO in-warranty hearing aid. The charge for the repair is to cover shipping, handling, and processing. It does not include the repair of the hearing aid as that is covered under the warranty.

If your hearing aid was purchased at Costco you will need to return to Costco for a repair. Hearing Aid Services PLLC cannot repair hearing aids bought from Costco.
