Out of Warranty Hearing Aid Repair Over 5 Years of Age (2 Hearing Aids)

PLEASE READ ENTIRE DESCRIPTION AND REPAIR FORM BEFORE SUBMITTING IT TO HEARING AID SERVICES PLLC.

This repair is for TWO out of warranty hearing aids over 5 years of age. The charge is to cover the repair, shipping, handling, and processing. If you are unsure of the warranty status or age of your hearing aid you can send an email to hearingaidservicespllc@gmail.com. Include the make, model, and serial number of your hearing aid.

Hearing aid repair has to be purchased before hearing aids can be repaired.

If your hearing aid was purchased at Costco you will need to return to Costco for a repair. Hearing Aid Services PLLC cannot repair hearing aids bought from Costco.
