Out of Warranty Repair Hearing aid Over 5 years of Age (1 Hearing Aid)

PLEASE READ ENTIRE DESCRIPTION AND REPAIR FORM BEFORE SUBMITTING IT TO HEARING AID SERVICES PLLC.

This repair is for ONE out of warranty hearing aid over 5 years of age. The charge is to cover the repair, shipping, handling, and processing.

Some parts and hearing aid models are discontinued after reaching 8-9 years of age. If your hearing aid is 8-9 years of age or older Hearing Aid Services PLLC may not be able to repair your hearing aid.

If you are unsure of the warranty status or age of your hearing aid you can send an email to hearingaidservicespllc@gmail.com. Include the make, model, and serial number of your hearing aid.

Hearing aid repair has to be purchased before hearing aids can be repaired.

If your hearing aid was purchased at Costco you will need to return to Costco for a repair. Hearing Aid Services PLLC cannot repair hearing aids bought from Costco.
